Susan Ngondo, APRN, is a family-practice nurse practitioner and founder of Afya Med Services LLC. Her experience includes primary and preventive care, medication management, chronic-care follow-up, and caring for adults and seniors with a range of healthcare needs.
Through Afya, Susan provides personalized care in private residences and in assisted-living, independent-living, and memory-care communities throughout Collier County and parts of Lee County. Afya is already actively serving residents in multiple senior-living communities in the Naples and Marco Island area, in addition to patients receiving care in their own homes and through telehealth.
Her approach emphasizes accessibility, continuity of care, medication management, communication with families, and coordination with senior-living communities and other healthcare professionals.
